How do I get more detailed information about the patient whose completed the Assessment and/or Module 1?
The information received will include their name, email, and contact number. For HIPAA reasons, we’re not able to transmit detailed information or results from their assessment. If you discover that they’re NOT an existing patient, please contact BaleDoneen.
When will I receive the packages that I’ve ordered?
Orders are taken care of by our fulfillment center. You can expect them in 5-7 business days.
How do I receive brochures?
As a Signature Member, you will receive 25 brochures when you order your first bundle. If you would like additional brochures, the PDF is available in the Member Portal.
How do I access the FULL program myself?

How do I receive the PowerPoint Presentation to use within my community?
The Community PowerPoint will be available in the Member portal under Marketing & Business. Select AHAforLife and the related documents will be displayed.
How do I receive the kits? Are they sent to me or my patients?
Kits will be sent to each Provider. It will be up to them to personally distribute OR mail to their patients.
Who should be offering the Program – Dentists, Physicians, or both?
